- Tax Identification Number (TIN) and Latest Income Tax Return (ITR)
- Community Tax Certificate (Cedula)
2. Additional Requirements for Locally Employed Applicants
- Certificate of Employment (COE) indicating salary, position, and tenure
- Latest Three (3) Months Payslips
